Please note the Medical Records Fax: (603) 663-6531 is for REQUESTING medical records ONLY, not for submitting medical records.

There are two ways to access your medical records. To maintain confidentiality and privacy, please choose one of the following options.

1) Request copies of your Medical Records

  • Complete and physically sign & date the form
  • Send the completed form to CMC using any of the following methods:
    • Fax: (603) 663-6531 (additional fax numbers: (603) 663-6915 or (603) 314-4722)
    • Email: CMCROI@cmc-nh.org
    • Mail: Medical Records, Catholic Medical Center, 195 McGregor Street, Suite 110, 1st Floor, Manchester NH 03102
    • Drop the form off in the lock box/slot at the CMC Medical Records office: 195 McGregor Street, Suite 110, 1st Floor, Manchester NH (the large, brick mill building across the street from the hospital—please use the South entrance)
  • If you have any questions, please contact CMC's Health Information Management Department at (603) 663-6516 and leave a message (Mon–Fri 8 AM–4 PM)

2) Patient Portal Access—access your medical information online anytime by signing up for the FollowMyHealth patient portal.

This web-based tool gives existing patients access to a secure, convenient and free way to manage personal health care information. Get 24/7 online access to FollowMyHealth from any computer, smartphone or tablet.

Through the portal you can:

  • Access your electronic medical information (for example: lab results, allergies, vital signs and other related health information)
  • Clinical documentation including nursing notes, flowsheets and procedural documentation
  • Keep a personal journal of your health information, such as home blood pressure readings or blood sugar levels
  • Send secure messages to your provider/healthcare team
