Patient transfer center specialty services
CMC Patient Transfer Center serves as a single point of access to highly-specialized care through a simple transfer process. We provide inpatient-to-inpatient and emergency-to-inpatient transfers from hospitals and facilities throughout the region.
Our advanced services include cloud-based diagnostic imaging to support decision-making, reduce radiation exposure and lower costs. Referring providers have secure access to our Patient Portal to monitor patients progress. Additionally, we provide quarterly activity reports for transferring facilities and maintain consistent communication between referring and receiving providers.

- Assistance with ambulance transport: Coordination through AMR, when needed.
- Patient placement support: Assistance with patient placement to quaternary care hospitals, when needed
- Highly specialized services: Tailored to meet patient needs
- In-house AMR representative: Arranges transportation back to community facility

- Once approved, our RN completes the transfer process
